To obtain a copy of the Engagement Letter Request Form to be completed and returned to us, simply click on the download link to the right. Once we receive your completed form, we will send you an Engagement Letter which details our fees and services as well as provides a laundry list of information that you will need to supply in order for us to perform the Practice Appraisal, which is the first step in the process. Our Engagement Letter requests certain information pertinent to an Associate Employment Agreement and the Purchase & Sale documents necessary for a Practice Transition. If you are merely requesting an Engagement Letter for the purpose of an Appraisal only, please disregard any requests for information related to the transition documents. Note: Once you return the information detailed in the Engagement Letter, you will have agreed to retain our services. |
